Good Days® ('CDFund' or 'CDF' or 'Good Days' or 'we' or 'us') is committed to protecting your privacy. This Privacy Policy applies to the personal information collected by us through this website (the 'Site'); it does not apply to other information collected through our other online or offline sites, products, or services.

Introduction

We understand that your medications are very important to you and your health. We are excited to bring you a resource that locates (the 'Services') financial assistance to pay for your medications as well as pharmacies to prepare your medications (collectively, the 'Suppliers'). After exploring our Site, we hope you will register as a participant. By registering, you provide us with the information we need to enable you to become an active participant on the Site and receive its related Services.

We are not providing medical advice, diagnosing, treating, or advising you about what providers to use. The Site is an informational resource for you, and we hope a valuable one. Decisions about your best lifestyle choices, what medications to take, how to take medications, and your medical treatments should be made in consultation with your healthcare providers, whom you choose based on your own needs.

Collection of Your Personal Information

The first time you sign into the Site, the Site will prompt you to create an account. To create an account you must provide personal information such as name, e-mail address and other contact information such as addresses and telephone numbers. You will also be prompted to enter information about yourself, including your diagnosis code, medical insurance information, medication type and dosage as well as your user name, password, physician's contact information, phone number, financial information, medical insurance information, and dependents. Certain insurance information must also be collected to determine the Suppliers to which your information will be submitted.

We will use the e-mail address you provide when you create your account to communicate with you regarding your account. It is your responsibility to let us know if this e-mail address changes. The Suppliers may also use your e-mail address as described in their privacy statements, which are the responsibility of those Suppliers.

How We Use and Share Your Personal Health Information

We use your personal information collected through the Site, including health information, to provide the Services as described in this Privacy Policy. In support of these uses, we may use personal information to provide you with important information about the Services and the Site, including critical updates and notifications.

The primary purpose of the Site is your ability to apply for assistance directly to Suppliers of financial aid and/or other assistance relating to your health condition. We will be sharing your personal information and health information when you submit inquiries for assistance to these Suppliers of financial aid and/or assistance.

We occasionally hire other companies to provide limited services on our behalf, such as answering customer questions about products and Services. We provide those companies only with the personal information they need to deliver the Service, such as IP address or e-mail address. We require these companies to maintain the confidentiality of the information they receive, and prohibit them from using the information for any other purpose.

We may access and/or disclose your personal information if we believe such action is necessary to: (a) comply with the law or legal process served on us; (b) protect and defend our rights or property (including the enforcement of our agreements); or (c) act in urgent circumstances to protect the personal safety and welfare of you and the other users of our Services or members of the public.

We do not sell or market your health information, and we do not share it with other individuals or services unless you explicitly authorize us to do so or in the circumstances described in this Privacy Policy. We do not use or disclose your information except as described in this Privacy Policy.

Personal information collected on the Site is currently stored and processed in the United States. Some Suppliers may store information in other countries, please read their privacy statements for more information regarding those Suppliers storing policies.

How We Use Aggregate Information and Statistics

We may use aggregated information from the Site for marketing of the Services (for example, to tell potential advertisers or sponsors how many Site users live in the United States) or for other purposes. This aggregated information is not associated with any individual account and does not contain personally identifiable information or individually identifiable health information. We do not use your individual account and record information from the Site for marketing without receiving your authorization.

Your Account Access, Changes and Controls

You choose whether or not to create an account with the Site. An account allows you to manage one or more requests for assistance for purposes of identifying and applying for potential Suppliers of assistance. You choose what information to put in your records. By default, you are the custodian of any records or information you create.

You can review and update your personal account information. You can modify, add, or delete any information by signing into your account and editing your personal account profile. The accuracy of your account information is your responsibility.

You can close your account at any time by sending an email through the Contact Us link. We wait 90 days before removing your account information from the Site in order to help avoid accidental or malicious removal of your information. The same mechanism can also be used to reopen an account that has been closed.

Third Party Account Access and Safeguards

In order to access the Site, the Suppliers must commit to protecting the privacy of your personal information. We can revoke a Supplier's access to the Site if a Supplier does not meet its privacy commitments to us. We encourage you to contact us if you believe a Supplier is not protecting the privacy or security of your personal information.

We require Suppliers to agree not to disclose your personal information without your express authorization, unless it is otherwise permissible by law.

E-Mail Controls

The Site may send e-mails advising you of changes and/or improvements to the Services or the www.gooddaysfromcdf.org website.

E-mails may also be sent to you if you forget your password. If you click on the 'forgot password' link, then after you enter your user ID, your password for the Site will be sent to the e-mail address on file for you.

Privacy and Security of Your Personal Information
